I've had ones that work perfectly following the original instructions and I've had ones that needed encoding as H.264 then converting in iTunes (see above). Not sure yet why it is different. However, when I tried to sync to my iPhone - no joy - everything was properly checked, but the file didn't sync. Next, I right clicked on the file inside of iTunes and selected "Convert selection to iPhone/iPod". Boom - it works! Thanks for the tips and troubleshooting.
